Job Description

• Provide support and assistance to aide the consumer utilize generic and natural resources available in the community (e.g. free social gatherings, discount events, coupons for shopping, food pantries, utility discount programs, advocacy services and etc)
• Provide supervision of the consumer while in the community and at home to achieve full inclusion.
• Support the consumer’s rights and informed choice, explaining consequences and assisting the consumer to seek the assistance of the program supervisor and other agency professionals for grievances, support to locate adaptive equipment, resources, discount programs and etc.
• Support and teach the consumer about maintaining safety and wellbeing: emergency preparedness, preventing infection, eating healthy, minimizing fire hazards, and minimizing trip hazards and other household dangers.
• Assist/monitor/teach the consumer to holding house meetings and informal discussion groups with peers.
• Teach and/or support the consumer to utilize public transportation. Transport the consumer to appropriate medical appointments (as directed).
• Perform daily record keeping (i.e. PA Charting, Medication Logs, Medical Visit Reports, Case Notes, Dietary Charting or others as specified by the program supervisor). Turn this documentation in at appropriate intervals, twice per month. Exchange information with the next shift and/or with the program supervisor.
• Teach relationship building skills.
• Teach community integration; assist consumers in joining community organizations of their choice.
• Teach problem solving skills/resolving conflicts.
• Encourage the consumer to participate in leisure/recreational activities fitting the consumer’s preferences and budget.
• Assist in creating natural supports for consumers.
• Assist consumers in building their self esteem, encourage self-actualization and empowerment.

Our Caregivers will perform non-medical homecare for clients who, due to advanced age, physical disability, and/or mental deficiency, need supervision. Caregivers also may work flexible schedules of their choice. These shifts can occur during the morning, day, evenings or overnight. Caregiver Responsibilities:
• Arrive to and from assigned shift on time and properly document all shifts worked
• Appropriately report any changes to client’s mental/physical health or home environment immediately
• Maintain proper documentation pertaining to client health and follow specific instructions pertaining to client health and other duties assigned
• Comply with rules and regulations pertaining to client health information, specifically HIPAA’s policies and procedures
• Adhere to all policies and procedures of 24Hr HomeCare
• Represent 24Hr HomeCare to the best of your ability
• Promote positive, supportive, and respectful communication to client/family and 24Hr HomeCare staff

Qualifications

• Professional caregiving experience, CNA graduate, or current LVN student w/ completion of clinical rotations
• 3 professional references (able to verify caregiving duties)
• Proof of negative tuberculosis test/chest x-ray
• Professional appearance and demeanor
• Two forms of ID compliant with I-9 requirements (ex. driver’s license, passport, social security card, etc.)
• Registered Home Care Aide (HCA) (CA only)
• Cleared criminal background
